The only movie goof I found in this great film, is that one of the psychic children, Masaru, his mouth disappeared at 2:46.

Actually, Tetsuo did not "die" in the end. Akira absorbed him and became part of Akira and created a new universe. In the end of the movie, Tetsuo is seen symbolically as a ball of light - a consciousness - in his own universe. He utters the word "I am Tetsuo" to tell us that he evolved. Bear in mind that the movie is based on the Buddhist theme of death and rebirth (reincarnation), and this is also represented in the movie's soundtrack which is inspired from Buddhist prayer chants.;)
